ABSTRACT

RESUMEN Objetivo: Estimar los valores morfológicos y morfométricos del endotelio corneal según la cantidad de células y evaluar la concordancia interobservadores para los diferentes parámetros, considerados según los diferentes conteos celulares en adultos sin alteraciones corneales. Métodos: Se realizó una investigación observacional, descriptiva y transversal de serie de casos en el Servicio de Cirugía Refractiva del Instituto Cubano de Oftalmología "Ramón Pando Ferrer" en dos años de estudio. Después de aplicar los criterios de exclusión, la muestra quedó conformada por 200 ojos de 100 pacientes adultos sin alteraciones corneales. Se realizó microscopia endotelial de no contacto SP-3000P, para identificar los valores morfológicos (hexagonalidad y coeficiente de variabilidad) y morfométricos (densidad celular), así como el promedio del tamaño celular corneal según cantidad de células evaluadas. Resultados: Según la cantidad de células endoteliales evaluadas, no existieron diferencias significativas de las variables morfológicas y morfométricas (p> 0,05) en ambos ojos. La concordancia entre los diferentes conteos celulares según los valores de los coeficientes de correlación intraclase fueron todos altos. La concordancia interobservadores (excepto para la hexagonalidad) y los coeficientes de correlación intraclase fueron altos. Conclusiones: Los valores morfológicos y morfométricos del endotelio corneal según cantidad de células evaluadas, son similares en todos los conteos celulares. Se demuestra una buena concordancia entre los diferentes conteos celulares estudiados para los diferentes parámetros estimados(AU)


ABSTRACT Objective: To estimate the morphological and morphometric values of the corneal endothelium according to the number of cells and evaluate the interobserver concordance for the different parameters, estimated according to the different cell counts in adults without corneal alterations. Methods: An observational, descriptive and cross-sectional case series research was carried out in the Refractive Surgery Service of Ramón Pando Ferrer Cuban Institute of Ophthalmology during two years of study. After applying the exclusion criteria, the sample was made up of 200 eyes of 100 adult patients without corneal alterations. Non-contact endothelial microscopy SP-3000P was performed to identify morphological values (hexagonality and coefficient of variability) and morphometric values (cell density), as well as the average corneal cell size according to the number of cells evaluated. Results: According to the amount of endothelial cells evaluated, there were no significant differences between morphological and morphometric variables (p>0.05) in both eyes. The agreement between the different cell counts according to the values of the interclass correlation coefficients (ICC) were all high. The interobserver concordance and ICCs were also high, except for hexagonality. Conclusions: The morphological and morphometric values of the corneal endothelium, according to the number of cells evaluated, are similar in all cell counts. Good concordance between the different cell counts studied for the different estimated parameters is demonstrated(AU)

ABSTRACT

Objective To investigate imaging characteristics of human skin in different sites by using 20-MHz and 50-MHz high-frequency ultrasonography,and to compare the reliability of skin thickness measurement by the above two approaches of ultrasonography.Methods A total of 39 healthy volunteers aged 18-39 years were enrolled into this study.Then,20-MHz and 50-MHz ultrasonography were separately performed to image 20 different sites on the body,and the thickness of the epidermis and dermis were measured and evaluated by 3 dermatologists independently.The ultrasonic images were analyzed,and the intraclass correlation coefficient (ICC) was used to assess and compare the reliability of skin thickness measurement by 20-MHz and 50-MHz ultrasound.Results The ICC values were less than or close to 0.7 in epidermal thickness measurement at almost all the tested sites between 20-MHz and 50-MHz ultrasonography,suggesting poor reliability.However,20-MHz and 50-MHz ultrasonography showed excellent reliability in dermal thickness measurement with the ICC greater than 0.75 at almost all the tested smooth and flat body sites.The 20-MHz ultrasound could provide clear images of the dermis and subcutaneous tissue,while 50-MHz ultrasound could only provide clearer images of the dermis in details.In some uneven sites with complex anatomical structures,20-MHz and 50-MHz ultrasound probes had their own advantages and disadvantages in imaging depth and detail resolution.Conclusions The 20-MHz and 50-MHz ultrasono-graphy both can serve as non-invasive imaging techniques to show structures of the epidermis and dermis better.In different parts of human body,ultrasound frequencies should be selected according to their imaging characteristics.

ABSTRACT

Este artigo avalia a confiabilidade teste-reteste do Índice de Capacidade para o Trabalho (ICT) em trabalhadores de enfermagem. Foi aplicado questionário autopreenchível duas vezes a um grupo de 80 trabalhadores de enfermagem (enfermeiros, técnicos e auxiliares de enfermagem) de um hospital público no Município do Rio de Janeiro, com intervalo entre sete e quinze dias. A confiabilidade foi estimada pela estatística kappa ponderada quadrática, pelo coeficiente de correlação intraclasse (CCI) e pelo gráfico de Bland e Altman. Dos participantes, 81% eram mulheres e com idade variando de 22 a 67 anos (média = 39,1; DP = 10,8anos) e 36,3% tinham ensino superior completo. O escore global do ICT apresentou CCI = 0,79 (IC95% 0,67 a 0,86) e kappa = 0,69 (IC95% 0,50 a 0,80) para o ICT categórico (classificado em baixo, moderado, bom e ótimo). O kappa dos itens do ICT variou de 0,39 a 0,82 e o gráfico de Bland e Altman não mostrou um padrão sistemático. A concordância entre as medidas de teste-reteste indica grau aceitável de confiabilidade, sugerindo adequação do processo de aferição entre trabalhadores de enfermagem.


This paper assesses the test retest reliability of the Work Ability Index (WAI) in nursing workers. A self-administered questionnaire was applied twice to a group of 80 workers (nurses and nursing aides/assistants) at a public hospital in Rio de Janeiro, Brazil within an interval from seven to fifteen days. The reliability was estimated using quadratic weighted kappa statistics, interclass correlation coefficient (ICC) and the Bland and Altman plot. Eighty-one percent of participants were women aged betweenfrom 22 to 67 years (mean =39.1; SD=10.8 years); 36.3% had completed higher education. The global score of the WAI presented ICC= 0.79 (IC95% 0.67 to 0.86) and weighted kappa=0.69 (CI95% 0.50 to 0.80) for categorical WAI (classified as low, moderate, good and excellent). The quadratic weighted kappa of the WAI items ranged from 0.39 to 0.82 and the Bland and Altman plot did not show a systematic pattern. The agreement between the test and retest measures shows an acceptable degree of reliability, suggesting the adequacy of the assessment process among nursing workers.
